What the inspector wrote

Licensing Program Analysts (LPA) Brian Balisi  conducted an unannounced  Case Management - Deficiencies visit in conjunction with a complaint visit (Complaint Control # 29-AS-20260415105855).  The purpose of the visit is to issue citations for deficiencies observed during the initial complaint investigation. Record review and interviews revealed that Staff #1 (S1) and Staff #2 (S2) had been working at the facility for at least 30 days but were subsequently disassociated on 03/02/2026 due to incomplete fingerprint submissions. The following deficiency was observed (See LIC 809-D) and cited from the California Code of Regulations, Title 22 and/or California Health and Safety Code. Civil penalty issued in the amount of $1,000. Exit interview was conducted. A copy of the report and appeal rights were provided. Administrator refused to sign report.
