What the inspector wrote

Residents are not provided adequate meals It is alleged that residents are not provided food of good quality and is not cooked properly. LPA's conducted interviews with residents from 10am-11:30am. Interviews reveal that the majority of residents are not happy with the food quality. They complained that the food was hard, overcooked, and bland. LPA conducted a physical plant walk through of the kitchen area to observe the food supply. Based on the information obtained through interviews this allegation is deemed Substantiated. Deficiency cited on LIC 9099 D. Exit Interview conducted. Appeal Rights explained. Copy of report emailed.

Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors.This requirement was not met as evidenced by: Based on interviews conducted it was found that the facility did have an issue with pest and that some rooms and parts of the facility were not clean and in good repair which posed a potential health and safety risk to residents in care.

This requirement is not met as evidenced by: Based on interviews conducted the facility has not had an activity director for over a year.
