What the inspector wrote

Licensing Program Analyst (LPA) Abeye Duguma met with Alexcis Paralta for a One (1) Year Required - Infection Control visit for this facility. LPA explained the reason for the visit. A tour of the physical plant was conducted at 3:00pm and the following was noted: There is one entrance being utilized at the facility, there are required posters posted at the main door. Screening area is located immediately upon entrance. Sign in sheet, infrared thermometer, hand sanitizer, gloves and masks are available. LPA was screened upon entry. All staff were observed to be wearing masks during the visit. Signs to wear masks and other COVID 19 prevention protocol are posted outside. Hand washing, coughing etiquette, physical distancing and other necessary signs are posted in the bathroom and throughout the facility. The facility has enough PPE supplies. The facility has a total of six (06) bedrooms and seven (07) bathrooms of which one is for both residents and staff. The facility is fire cleared for twelve (12) non-ambulatory of which twelve (12) may be bedridden and a hospice waiver for twelve (12). The facility is currently occupying six (06) ambulatory, four (04) non-ambulatory residents of which one (01) is bedridden and under hospice care. The facility has outdoor furniture with a covered shaded area for residents and visitors. The facility does not have a swimming pool/body of water. Laundry detergents, cleaning agents and other toxins are locked away. Kitchen is sufficiently stocked with at least two (2) days perishable and seven (7) days non-perishable food. Frozen foods are wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. Knives and sharps are observed to be locked and inaccessible to residents. Living and dining room furniture were also checked. The living and dining room are neat and clean. The facility maintains a comfortable temperature at 73°F. The smoke and carbon monoxide detectors are hardwired, interconnected and observed to be operational. Fire extinguisher is located near the dining area, observed to be full and last inspected on 06/17/2021. (continued on LIC 809-C) The residents' rooms are adequately furnished with appropriate lighting system. Hallways are well lit. Residents have enough personal hygiene product provided by the licensee. The bathrooms were checked for cleanliness and proper operations. The hot water temperature was measured at 117.2°F. Towels and washcloths are not shared. There was enough clean linen available in the linen cabinet. LPA observed medication and first aid kit to be locked and inaccessible to residents. Exit interview conducted. Copy of this report issued.

The activities made available shall include: (1) Socialization...(2) Daily living skills/activities...(3) Leisure time activities...(4) Physical activities..(5) Education...(6) Provision for free time...This requirement was not met as evidenced by: Licensee did not provide planned activities to residents in care. LPA interviewed three (03) out of seven (07) residents who revealed that they are bored due to the lack of activities. LPA did not observe any resident engaged in any activity or see an activity calendar posted in the facility which poses a potential health and safety risk to residents in care.

    87468.1(a)(2)-Personal Rights-Residents in all residential care facilities for the elderly shall have the following personal rights-(2)-to be accorded safe, healthful, and comfortable accommodations, furnishings, and equipment.This requirement was not met as evidenced by: Based on observation, record review, and interviews facility failed to ensure that residents in care are accorded a safe accommodation as seven (07) out of seven (07) residents were left without supervision while staff came in and out of the facility at various times which poses an immediate health and safety risk to 7 out of 7 residents.
