On 12/17/2025, Licensing Program Analyst (LPA) Regina Cloyd conducted a case management - health checks. LPA met with House Manager Evelyn Allego and spoke with Administrator Antonia Dionisio over the phone. The facility is licensed for six residents and approved for two hospice residents. LPA toured the facility and four out of four residents were present, alert, and greeted LPA Cloyd. Regarding the status of the change of ownership, the Administrator indicated that she submitted missing paperwork to Centralized Application Bureau (CAB) towards the end of October or the beginning of November. Administrator to provide contact information for her CAB Analyst. LPA reminded Administrator to pay outstanding fee associated with a fire clearance violation, CCR 87202(a), at the annual inspection on 08/07/2025. No deficiencies observed today. An exit interview was conducted and a copy of this report was provided to the House Manager Evelyn Allego.
