On 01/17/2026, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced required – annual inspection and met with Staff , Wellness Director Michelle Brown, and spoke with Executive Director Brittany Kavanaugh. During today's visit, LPA toured the facility and tested water temperatures in eleven resident bathrooms, reviewed eight resident records, menu, activities schedule, observed lunch and social activities, tested signal system, interviewed the Chef, reviewed nine staff records, emergency disaster plan, infection control plan, plan of operation, liability insurance, and observed dinner. Due to insufficient time, an annual continuation is required. An exit interview was conducted and a copy of this report was left with staff and emailed to the Executive Director Brittany Kavanaugh.
