Water supplies and plumbing fixtures shall be maintined as follows: (2) Faucets used by residents for personal care such as shaving and grooming shall deliver hot water. Hot water temperature controls shall be maintained to automatically regulate the temperature of hot water used by residents to attain a temperature of not lee than 105 (degrees F (41 degree C) and not more than 120 degrees F (49 degree C).This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above. In all bathrooms water temperature is at 100.4 degrees F, which poses an immediate health, safety or personal rights risk to persons in care.

Incidental Medical and Dental Care.(a) A plan for incidental medical and dental care shall be developed by each facility...(5) The licensee shall assist residents with self-administered medications as needed.This requirement is not met as evidence by: Based on observation administrator did not ensure that R1's medications are listed in the MAR. R1's Folic Acid 1 mg and Aspirin EC 81 MG were present at the facility / prescribed by the doctor, but not listed / recorded on the MAR.

During medication review, LPA observed Resident #1 had Vitamin D3 and Calcium D3 medications which were not labeled. LPA also observed Resident #2 had "Kirkland" Aspirin which was not labeled. This poses a potential health, safety, and/or personal rights risk to the residents in care.

During Physical Plant tour, LPA measured water temperature in bathroom #1 and bathroom #2 and water temperature measured between 101.7F - 101.8F which is below the required 105F - 120F.
