Follow-up on corrections·June 2, 2023

On 06/02/23, Licensing Program Analyst (LPAs) Ernand Dabuet and Alfonso Iniguez initiated a Case Management visit. LPA was met by Wilfred Guerrero Resident Services Director and explained the purpose of today’s visit. During a complaint investigation visit on 06/02/23, LPAs observed the facility did not have a current Executive Director on file and is not able to provide a Designation of Facility Responsibility LIC 308 for to Community Care Licensing (CCL) for review. According to Guerrero, the facility has not had an Executive Director for a long time. The facility has on record Terri Weitzman who is the active administrator and is not in compliance with PIN 21-24 CCLD. According to the California Code of Regulations (Title 22, Division 6, Chapter 8), the following deficiency has been observed and citation issued (ref. LIC 9099-D). An exit interview conducted with Wilfred Guerrero and a copy of report and appeal rights provided. Note: *Citations not cleared by the due date will be a $100 fine assessed for each citation until it is cleared. Civil penalties will continue to accrue until Proof of Corrections (POC) are cleared. *
