    Observation of the Resident: The licensee shall ensure that residents are regularly observed for changes in physical, mental, emotional, and social functioning and that appropriate assistance is provided when such observation reveals unmet needs. When changes such as unusual weight gains or losses or deterioration of mental ability or a physical health condition are observed, the licensee shall ensure that such changes are documented and brought to the attention of the resident's physician and the resident's responsible person, if any. This requirement is not met as evidenced by: Resident #1 was diagnosed with a Stage III (2cm x 3cm) pressure injury to the right foot, back of heel on 12/31/22; Stage III (2cm x 2cm) pressure injury to back of right heel on 01/03/22; Stage III (1.5cm x 1.5cm) pressure injury to the right heel on 01/05/22 and (again) on 01/10/22; and, the resident was not taken to the hospital until 01/17/22. This violation which posed an immediate health, safety or personal rights risk to persons in care.

This violation which posed an immediate health, safety or personal rights risk to persons in care.
