What the inspector wrote

LPA Loera arrived unannounced to conduct a case management visit. LPA was greeted by Administrator, Cleda Odiwe and discussed the purpose of the visit. LPA followed up on an incident report that was sent to Community Care Licensing (CCL) on 06/09/2025. Incident report states on 06/06/2025 around 6:10pm, caregivers on duty noticed resident (R1) was nowhere to be found. Staff looked for R1 around the facility but was unsuccessful. Administrator called Novato Police Department and R1's responsible party to tell them about the incident. A woman driving along Carmel Drive found R1 walking along the street and immediately called 911. Per R1's physicians report (LIC602) dated 02/14/2024, R1 has dementia and can leave unassisted. Record review shows R1 does not have an updated physicians report (LIC602) as their last medical assessment was over 12 months ago. (Deficiency Cited, 87463(h)) . Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment. Exit interview conducted. Copy of report, LIC9099D (Deficiency Page), Plan of Corrections, and Appeal Rights discussed and provided to Administrator.
