What the inspector wrote

On 10/17/2025 Licensing Program Analyst (LPA) B. Miranda arrived to the facility unannounced to conduct an annual inspection. LPA met/spoke with staff Maria Viramontes and was allowed entry into the facility. LPA requested for staff to call Administrator to inform LPA had arrived to conduct annual inspection. Administrator was contacted and arrived shortly after. When LPA arrived they observed the facility to be clean, clutter free, and odor free. LPA observed R3 sitting at the table eating breakfast. LPA reviewed facility roster and verified staff is current are showing cleared on roster. LPA reviewed staff and resident files which are current and up to date. Fire extinguisher was last serviced 11/19/2024 and still holds charge. Smoke detectors and carbon monoxide readers were tested and in operating order. LPA tested the water in the kitchen which read at 105.4 degree Fahrenheit. LPA observed chemicals, a knife, and tools were not locked and not properly secured in kitchen. LPA also observed rotting strawberries and zucchini in the refrigerator along with expired macaroni salad. LPA also observe laundry detergent and bleach on a shelf above the washing machine which was not locked. LPA observed resident rooms to be properly furnished with adequate lighting. Two residents were at the facility and the third resident was a day program. LPA observed medication was locked and inaccessible to residents. Medication count was verified and correct. Centrally Stored Medication log was correct at this time. Exit interview was conducted and a copy of this report LIC809, LIS809Ds, & appeal rights were provided to Licensee Maria Tostado.

LPA observed R1’s physical it states the resident’s safety would be at risk if they had access to disinfectants, cleaning solutions, poisonous substances, knives, matches, tools, sharp objects, and other similar items. LPA observed laundry detergent & bleach above the washer to be accessible, a knife in an unlocked kitchen drawer, cleaning products in and unlocked cabinet under the sink, various tools in an unlocked drawer in the kitchen, scissors in kitchen drawer, antifungal cream in an unlocked drawer & toilet bowl cleaner under the sink in the hallway bathroom.
