Based on observation, interview, & record review, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. LPA observed R1’s physical it states the resident’s safety would be at risk if they had access to disinfectants, cleaning solutions, poisonous substances, knives, matches, tools, sharp objects, and other similar items. LPA observed laundry detergent & bleach above the washer to be accessible, a knife in an unlocked kitchen drawer, cleaning products in and unlocked cabinet under the sink, various tools in an unlocked drawer in the kitchen, scissors in kitchen drawer, antifungal cream in an unlocked drawer & toilet bowl cleaner under the sink in the hallway bathroom.

LPA observed strawberries & zucchini to have mold. LPA observed bacon and sausage stored in plastic baggy with label or date. LPA observe expired macaroni salad in refrigerator.
