What the inspector wrote

Licensing Program Analysts (LPAs) Daiquiri Boyd and Shawna Doucette made an unannounced visit to the facility to conduct a case management visit. LPAs were met at the door by Administrator, Johnson Wanjau. LPAs requested a tour of the facility. During the tour, LPAs noticed that the facility had new construction in the garage area. There was a kitchenette, two bedrooms and a dining/living area. Refer to 809D. Civil Penalty was issued for fire clearance. A copy of this report was provided with plan of correction and appeal rights.

Prior to accepting or retaining any of the following types of persons, the applicant or licensee shall notify the licensing agency and obtain an appropriate fire clearance approved by the city, county, or city and county fire department, or district providing fire protection services, or the State Fire Marshal. This requirement was not met as evidenced by Licensee changed the garage into living apartment for staff without notifying the Department or obtaining fire clearance; which poses an immediate risk to the health, safety, or personal rights risk to the residents in care.
