What the inspector wrote

On 11/14/24, Licensing Program Analyst (LPA) V Gorban arrived to the facility unannounced to conduct a case management visit. LPA met with Administrator Natasha Prunty and explained the reason for the visit. During this visit LPA toured the facility, observing residents in care performing safety check. Based on interview and records review the facility had incident on October 26 th, 2024 that resulted in water shut off for period of couple hours. Incident was not reported by SIR to Licensing Regional office, Fresno. This incident is cited on attached LIC 809-D
