This requirement was not observed by: The facility failed to maintain bathroom floor clean and sanitary. Water leaks from the ceiling and wet floors created potential hazard. This requirement was not observed as evidenced by: Based on interview and records review the facility failed to ensure R1 medication as prescribed. R1 took wrong medications ( dose of Mitrazapine and Risperidone instead of prescribed Ramipril) which poses potential health and safety risk to persons in care.

This requirement was not met as evidenced by: Based on records review the facility staff (S1) shared personal inappropriate video content of him with another individual with residents (R1 and R2) in care. This poses potential health and safety risk to persons in care.
