What the inspector wrote

Licensing Program Analyst LPA Samer Haddadin conducted an unannounced visit for the purpose of completing a case management deficiency visit. LPA was met by Executive Director, Dillon (ED). LPA was granted entry to the building and then explained the reason for the visit. On November 21, 2024, Community Care Licensing received an incident report from this facility regarding resident (R1) who eloped the building and was found by few good Samaritans. Today, LPA Haddadin visited the facility to review (R1) physician report in which it was determined that the resident was unable to leave the facility unassisted per medical record. Based on the observation made today, deficiencies are being cited per California Code of Regulations, Title 22, Division 6, Chapter 8. An exit interview was conducted and a copy of this report as well as appeal rights were discussed and provided with facility representative.
