What the inspector wrote

LPA Samer Haddadin conducted a case management visit on this date in response to a Special Incident Report (SIR) received on September 27, 2025. Upon arrival, LPA was greeted by Administrator Dillon Cagulada, who granted entry. The purpose of the visit was explained. The report indicated that Resident 1 (R1) experienced a change of condition and was transferred to the hospital. LPA reviewed R1’s physician’s report and hospital discharge paperwork. Documentation confirmed that the facility did not neglect R1 and that medical services were provided on the same day the change of condition occurred. Based on the information obtained, no deficiencies are being cited at this time. An exit interview was conducted, and a copy of this report was provided to the facility .
