Based on the information gathered during the investigation and review of documents obtained, LPA is unable to ascertain if the allegation occurred as reported due to conflicting information. R6 stated that the facility provided her daughter with a copy of her Admission Agreement. During the course of the interviews with staff, S1 reported that R1's son was notified that he was not the Responsible Party (RP) for R1; therefore, he could not get a copy of the Admission Agreement.
