Keep administrator certification or recertification records

Based on observation, interview, and record review, the administrator did not have a current valid Administrator's Certificate which poses a potential health, safety or personal rights risk to persons in care. LPA observed that administrator's name is not in the 'Pending Renewals Applications' list or 'Active Certificates List'. Administrator reported she is in the process of renewing her certificate.

Based on observation and record review, two out of five residents (Resident #3 and Resident #5) did not have valid medical assessments from 2025, which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and record review, Staff #1 and Staff #2 did not have valid CPR/First Aid certifications, which poses a potential health, safety or personal rights risk to persons in care. LPA observed that both First Aid certifications expired in 2025.

Maintain facility in clean, safe, sanitary condition

Based on LPA's observation during facility tour, one stove burner was not working.

Based on LPA's review of facility records one staff does not have documentation of current year staff training. This could pose a potential health and safety risk to residents in care.

Based on review of resident records, residents with dementia did not have updated medical reports. This could pose as a potential health and safety risk to residents in care.

Maintain physician order documentation in resident record

Based on observation and interviews, facility did not maintain a written doctor's order for the half rail for one resident which poses an immediate Health, Safety, or Personal Rights risk to persons in care.

Based on observation, two out of the four bathrooms measured at 128.0 and 129.4 degrees Fahrenheit which poses an immediate Health, Safety, and Personal Rights risk to persons in care.
