What the inspector wrote

Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced case management visit in conjunction with complaint visit 22-AS-20230823105523. LPA was greeted and granted entry into the facility and explained the reason for the visit. Administrator Jonathan Martinez arrived during the visit. During the course of the complaint investigation, LPA reviewed training records for staff. Three out of three staff do not have current training records in the file. Based on the observations made from today's visit, deficiency is being cited per Title 22 Division 6 of the California Code of Regulations. This report was discussed with the Administrator and a copy was emailed to Administrator as well as Appeal Rights.

This poses a potential health and safety risk to residents in care.

    Licensees shall maintain in the personnel records verification of required staff training and orientation. This requirement is not being met as evidenced by: Based on observation, Licensee failed to ensure proof of training is maintained in the personnel file. Three out of three staff do not have current training in the file. This poses a potential health and safety risk to residents in care.
