What the inspector wrote

Licensing Program Analysts (LPAs) Kimberly Lyman and Sam Haddadin conducted an unannounced case management in-conjunction with complaint #22-AS-20240821154034. LPAs were greeted and granted entry into the facility and explained the reason for the visit. During the course of the complaint investigation, LPA interviewed Administrator and witness. Witness indicates the admission agreement/ file for Resident 1 was requested on or about 08/12/2024. The records were produced by facility on 08/20/2024. Based on the observations made during today's visit, the following violation is being cited per California Code of Regulations, Title 22, Division 6, Chapter 8. An exit interview was conducted and a copy of this report as well as appeal rights were discussed and provided with facility representative.
