What the inspector wrote

On Friday May 1, 2026, Licensing Program Managers (LPMs) Laura Munoz and Troy Ordonez, and Licensing Program Analyst (LPA) Melissa Parks arrived at the facility unannounced. Present at the facility was Licensee Petre Mile and one staff member. The purpose of this visit was to issue the Temporary Suspension Order and immediate exclusion for S1 which will be in effect on Monday May 4, 2026 at 5:00pm. LPM Ordonez reviewed the temporary suspension order and LPM Munoz reviewed the immediate exclusion with Licensee Petre Mile. Copies of the accusation were provided to all residents. The Licensee was reminded that he must cooperate with the Department, local placement, and advocacy agencies in the onsite evaluation of the residents and their transfer to alternative placement sites. As a result of today’s visit, no deficiencies are being issued. Exit interview. Copy of report was left at the facility.
