What the inspector wrote

On Monday May 4, 2026, Licensing Program Analyst (LPA) Melissa Parks arrived at the facility. LPA Parks met with Licensee Petre. This visit is in regard to the Temporary Suspension Order and Immediate Exclusion which will be in effect at 5:00pm today. LPA Parks conducted a tour of the facility to ensure the health and safety of residents in care. As of 2:30 p.m., R1-R5 were successfully relocated, and no residents remained in care. The Regional Office staff ensured that the transition process was carried out in accordance with Title 22 regulations. Collaboration with the resident families and admitting facilities supported a smooth relocation for the residents, safeguarding their health, safety, and personal rights throughout the process. The Department emphasized that the Temporary Suspension Order and Immediate Exclusion remain in effect and that the licensee retains the right to appeal under the Notice of Defense procedures. see 809D for deficiencies cited during todays visit. A copy of this report and appeal rights were provided to the facility.
