This posed an immediate health and safety risk to residents in care.

Based on LPA Delgado's observation, the licensee did not comply with the section cited above in enclosed area, there was a gallon of Clorox bleach sitting inside a cabinet, Ajax was under the sink inside bathroom #2 that was not secured, locked and accessible to clients which poses an immediate health, safety or personal rights risk to persons in care.

Based on LPA Delgado's observation and interview, the licensee did not comply with the section cited above in the kitchen sink appears to be dirty with molding on base of faucet, two (2) pilot lights are not working; staff needs to use a match to turn on, the 2nd bathroom shower has an odor, dirty and baseboard leading into shower is rotting which poses/posed a potential health, safety or personal rights risk to persons in care.

This requirement was not met as evidenced by: The licensee did not ensure R1 was free from neglect. Based on interviews and record review, facility staff neglected the wounds of R1 which resulted in the development of maggots on R1's leg wounds. This poses an immediate health, safety and personal rights risk to residents in care.

Maintenance and Operation- (a) The facility shall be clean, safe, sanitary and in good repair at all times. Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors. This requirement was not met as evidenced by: Based on LPA's observations, the licensee did not ensure the faciltiy was maintained in a clean manner. LPA observed the stove top, microwave, and kitchen wall behind the stove to be splattered with grease. LPA also observed flies in the facility and a fly swatterbeing stored in a very easily accessable(cont in right column)
