Inspector’s narrative

What the inspector wrote

On 6/27/2023, Licensing Program Analyst (LPA) Chinwe Nwogene conducted an unannounced visit to investigate the above allegations. LPA met with Licensee, Tyla Kincherlow and explained the purpose of the visit. At the time of visit, LPA interviewed staff, residents and witness and made observations. Regarding the allegation, “Staff left residents unsupervised with minor for an extended period of time”, it was alleged a minor was allowed to provide care and supervision to resident alone for an extended period of time. LPA interviewed residents who denied hearing or seeing a minor provide care and supervision to residents. LPA interviewed Licensee who denied hiring a minor or leaving a minor alone at the facility to provide care and supervision to residents. LPA interviewed witness who denied seeing a minor providing care and supervision to residents at the facility. Regarding the allegation “Staff was under the influence while caring and supervising residents in care”, It was alleged staff is intoxicated when providing care and supervision to residents. LPA interviewed staff who denied being under the influence while providing care and supervision to residents. LPA interviewed residents who denied hearing or seeing staff under the influence when providing care and supervision to residents. LPA interviewed witness who denied seeing staff under the influence when providing care and supervision to residents at the facility. During the interview, LPA observed staff was not under the influence. Staff was in good behavior and was able to hold conversation. Based on LPA’s observation, interviews with staff, and residents, there is not enough evidence to support the above allegations. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated at this time. An exit interview was conducted, and a copy of this report was reviewed with and provided to Tyla Kincherlow.
