(c) A facility shall conduct a drill at least quarterly for each shift. The type of emergency covered in a drill shall vary from quarter to quarter, taking into account different emergency scenarios. This requirement is not met as evidenced by V1 not being associated to the facility which poses an immediate health, safety and personal rights risk to persons in care.

On and after July 1, 2015, all residential care facilities for the elderly, except those facilities that are an integral part of a continuing care retirement community, shall maintain liability insurance covering injury to residents and guests in the amount of at least one million dollars ($1,000,000) per occurrence and three million dollars ($3,000,000) in the total annual aggregate... LPA observed R1 has been in the facility since November 2023 and R2 has been in the facility since February 2023, the facility does not have a Physician's Report on file for R1 and R2.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in 1 out of 4 resident PRN medication not being documented and maintained which poses/posed a potential health, safety or personal rights risk to persons in care.

Criminal Record Clearance: (e) All individuals...shall prior to working, residing or volunteering in a licensed facility: (3) Request and be approved for a transfer of a criminal record exemption... This requirement was not met as evidenced by: Based on observation and interview, the Licensee did not comply with the above regulation with at least one staff (S1). LPA Colvin learned that S1 is the acting Administrator for the facility, and requires an Exemption Transfer to this facility. This is an immediate safety risk to all residents in care.
