  • Provide resident hot water for personal care

    MAINTENANCE & OPERATIONS: Hot water provided for the use of residents shall be maintained between 105-120 degrees F. The hot water measured 95.4 degrees F. This poses a risk to the Health & Safety of the residents in care.

    Maintain facility in clean, safe, sanitary condition

    (a) The facility shall be clean, safe, sanitary and in good repair at all times. Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors. This requirement was not met as evidenced by: Based on observation, Licensee failed to ensure facility is in good repair. LPA observed leaking shower head, and shower handle valves need to be replaced per AD. This poses a potential health and safety risk to residents in care.

    Based on LPA Delgado's observation, interview and record review, the licensee did not comply with the section cited above in S1 did not have criminal background clearance and was not associated to the facility; document revealed fingerprints were done on 8/6/2024 which poses an immediate health, safety or personal rights risk to persons in care.

  • Maintain records of centrally stored medication dosages

    Based on LPA Delgado and Perez's observation, interview and record review, the licensee did not comply with the section cited above in medications administration record for R1 and R2 is not being documented on each day being dispensed which poses/posed a potential health, safety or personal rights risk to persons in care.

  • Keep prescriptions in original containers

    Incidental Medical and Dental Care: (h) The following requirements shall apply to medications which are centrally stored: (5) Each resident's medication shall be stored in its originally received container. No medications shall be transferred between containers. Based on LPA observation of pre-poured cups containing an assortment of medication, Licensee did not follow the regulation. This poses a potential health and safety and personal rights risk to residents in care.

  • Submit and maintain current mailing address

    Criminal Record Clearance:(e) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1569.17(b) shall prior to working, residing or volunteering in a licensed facility:(1) Obtain a California clearance or a criminal record exemption as required by the Department or Based on LPA's record review, and interview, LPA found that S1 did not have a background clearance. Licensee did not follow regulation. This is an immediate health and safety and personal rights risk to residents in care.
