87506 Resident Records(a) The licensee shall ensure that a separate, complete, and current record is maintained for each resident in the facility or in a central administrative location readily available to facility staff and to licensing agency staff.This requirement is not met as evidenced by: Licensee agrees to up complete and up date 5 out 6 resident files and notify LPA Lee by POC date 09/08/2023 by 5:00 PM by end of day. LPA lee gave administrator a copy of what is missing in the 6 resident's file from LIC 858 on a word document.

Maintain facility in clean, safe, sanitary condition

Based on observation and interview, the licensee did not comply with the section cited above. Licensee did not ensure resident room #4 was in good repair. There's a broken intercome lose coming out of the wall with exposed wire, which poses an immediate health, safety or personal rights risk to persons in care.
