What the inspector wrote

Licensing Program Analysts (LPAs) Talwinder Bains and Lavinia Muscan arrived at the facility unannounced on 5/14/25 to do case management visit . LPAs met with Administrator , Susie Dizon and explained the purpose of the visit. Incident: Department conducted a case management visit to follow up on an incident which happened on 5/9/25 as reported by the facility. Incident report stated that: on May 08, 2025 at approximately 2:20pm, an unidentified adult female forcibly entered the facility premises through climbing a fence and through the back door. The individual appeared agitated and aggressive. They proceeded to cause property damage by by breaking back door and window(s) located in room 105 and room 106. Facility staff Immediately followed emergency protocol. Law enforcement was contacted at 2:20pm and facility administrators was notified. The individual was detained in the front yard of the premises after causing the damage when authorities arrived. A police report was filed, and officers conducted a search. Facility notified all residents, families, responsible parties, and all other required agencies regarding this incident. Facility followed emergency protocol to handle this incident and conducted staff training. No immediate health and safety concerns were identified related to residents during todays visit. No citations were issued per Title 22 Regulations. Exit interview conducted and copy of the report left at facility.
