What the inspector wrote

On 07/23/25, Licensing Program Analyst (LPA) Talwinder Bains conduced a Case Management visit at the facility. LPA met with Administrator, Maria Susie Dizon and explained the purpose of this visit. Facility submitted LIC624 ,Incident Report (IR) on 07/21/25 regarding resident, R1 who had change in health condition on 07/21/25. IR reflected that R1 was evaluated by staff with altered mental status on 07/21/25 and staff called emergency services so R1 can be seen at local hospital to get appropriate medical care. R1 refused to go with emergency staff and left the facility on foot. Facility called the law enforcement to ensure the safety of R1. IR indicated that R1 told everyone that they do not want any treatment and left the facility at thier will. Facility was told by emergency response staff and by law enforcement that R1 cannot be forced to be seen by hospital staff against their will/consent. Facility notified LTCO ,responsible party and other required agencies regarding this matter on 07/21/25. LPA learnt that R1 did not return to the facility after this incident and staff were not aware about R1s whereabouts. At this time, this incident is under review and department will do follow up if warranted. No citations were issued per Title 22 Regulations. Exit interview conducted and copy of the report left at facility.
