Update reappraisal at required intervals

97463(a) The pre-admission appraisal shall be updated,.. to keep the appraisal accurate......when there is significant change in the resident’s condition, or once every 12 months, whichever occurs first.. This requirement was not met as evidenced by: Based on interviews and record reviews, the licensee did not update the appraisal when the resident began to wander during the first week of placement.

1569.321(e) Every facility...shall provide at least the following basic services: (e) Monitoring the activities of the residents while they are under the supervision of the facility to ensure their general health, safety and well-being. Based on interviews and record reviews, the licensee did not meet the supervision needs of clients. This poses a potential Health, Safety or Personal Rights risk to persons in care.

87705(b) Care of Persons with Dementia(b) Licensees who retain residents with dementia ... shall have at least one night staff person awake and on duty if any resident with dementia is determined ... to require awake night supervision. Based on interviews and record reviews, the licensee did not meet the supervision needs of dementia clients in need. This poses a potential Health, Safety or Personal Rights risk to person in care.

Limit staff assistance with self-administered drugs

The licensee shall assist residents with self-administered medications as needed. The requirement is not met as evidenced by: Based on record revew and interviews, licensee did not ensure that R1 received medications as prescribed since 3/25/21. This poses an immediate health and safety risk to residents in care.

Safe, healthful, comfortable accommodations

Personal Rights of Residents in All Facilities (a) Residents in all residential care facilities for the elderly shall have all of the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment.This requirement is not met as evidence by: Based on observation and interview the licensee did not protect the personal rights of clients in care to receive safe and healthful accommodations and engaged in conduct inimical to the health, welfare, and safety of clients in care, in that S1, S2, and S3 did not wear face coverings while providing care and supervision to clients in care, in violation of official government orders requiring the wearing of face coverings while working under specified conditions which poses an immediate health and safety risk to residents in care.
