Inspector’s narrative

What the inspector wrote

On 7-21-23 at 3:00pm, Licensing Program Analysts (LPAs) Michael Bilger and Arvin Villanueva arrived unannounced to continue the investigation for the allegation noted above. LPAs met with Administrator Caleb Summerhays and explained the purpose of the visit. During the investigation for this allegation, LPAs conducted interviews which included three staff members and 3 residents in care. Based on interviews conducted, it was determined that Resident1 (R1) was receiving phone calls, however, misplaced phone in the process as well. It was further determined through interviews that residents in care are able to receive and make phone calls sufficiently. Additionally, based on interview and facility observation conducted on 7-21-23, it was determined residents in care receive phone calls via communication between receptionist and other staff members. As a result, the preponderance of evidence standard is not met, and this allegation is UNSUBSTANTIATED. An exit interview was conducted with Caleb Summerhays and a copy of this report was left with Caleb. Appeal rights provided.

    87465 Incidental Medical and Dental Care (a) A plan for incidental medical and dental care shall be developed by each facility...(4) The licensee shall assist residents with self-administered medications as needed.This requirement was not met as evidenced by: Based on interviews and record reviews, licensee did not ensure that medications were given as ordered consistently for R1 during December 2022. This posed an immediate health and safety risk to residents in care.

    1569.312 Basic services requirement. Every facility required to be licensed under this chapter shall provide at least the following basic services: (e) Monitoring the activities of the residents while they are under the supervision of the facility to ensure their general health, safety, and well-being. This was not met as evidenced by: Based on interviews and record reviews, the facility licensee did not ensure care and supervision (including regularly checking residents) to R1 while in care at the facility which resulted in R1 sustaining multiple injuries consistent with a fall and was left unattended for a extended period of time.
