What the inspector wrote

On 7/21/23 at 4:00 PM Licensing Program Analysts (LPAs) Michael Bilger and Arvin Villanueva arrived at the facility unannounced to conduct a case management visit regarding medications. LPAs met with Caleb Summerhays and explained the purpose of the visit. LPAs reviewed medication administration record (MAR) of resident 1 (R1) dated December of 2022. LPAs interviewed Staff 1 (S1). Based on interviews and record reviews, it was determined that R1 was not assisted with self administration of medication on various dates on December 2022. Additionally, it was determined that medications were not consistently given as ordered on various dates during the month of December 2022. Furthermore, it was revealed that a plan was not in placed to rectify medications which were not available as a result of untimely response to pharmacy. As a result of this case management, deficiencies were cited under Title 22, Division 6, Chapter 8. Exit interview was held and a copy of report and appeal rights were provided to the facility.

This posed an immediate health and safety risk to residents in care.

Every facility required to be licensed under this chapter shall provide at least the following basic services: (e) Monitoring the activities of the residents while they are under the supervision of the facility to ensure their general health, safety, and well-being. This was not met as evidenced by: Based on interviews and record reviews, the facility licensee did not ensure care and supervision (including regularly checking residents) to R1 while in care at the facility which resulted in R1 sustaining multiple injuries consistent with a fall and was left unattended for a extended period of time.
