On 7/31/2025 Licensing Program Analyst (LPA) Shakaricka Hughes, conducted a case management visit to the facility. LPA spoke with the administrator Veniana Banuve. The current census is 6 with 1 facility staff. LPA conducted a tour of the facility, and it was observed clean, and in good repair. LPA interviewed 4 out of 6 residents in care. During in interview with the facility administrator, she has started the process with the Technical Assistance Program (TSP). Administrator has started reassessing and updating the files of the residents in care. LPA observed a new care staff present in the facility, LPA checked the background clearance and association of the facility staff in Guardian, facility staff was cleared and associated to the facility. LPA spoke with the administrator about reporting requirements for incident reports submitted to CCL, administrator understands the process and accuracy required when submitting incident reports. Administrator states that training for new care staff has started, and when training is completed administrator will send proof of training to LPA. An exit interview was conducted, and a copy of this report was provided to facility at the end of this visit.
