    Based on observation, the licensee did not comply with the section cited above. The hot water temperature in both bathroom faucet were measured between 150 and 151 degrees Fahrenheit, which poses an immediate health, safety or personal rights risk to persons in care.

    Based on observation the licensee did not comply with the section cited above. Walkway to the only exit gate at the side was obstructed by items such as wood bookshelf/cabinet, shower chair and a large plant pot. Someone with wheelchair would not be able to fit which poses/posed a potential health, safety or personal rights risk to persons in care.

    All kitchen areas shall be kept clean and free of litter, rodents, vermin and insects. This requirement is not met as evidenced by:Based on observation, the licensee did not comply with the regulation cited above. During inspection of the kitchen and pantry, LPA observed cobwebs and debris inside the pantry and debris in kitchen cabinet where they store pots and pans. This poses a potential safety, health, personal rights risks to persons in care.

    All food shall be of good quality. Commercial foods shall be approved by appropriate federal, state and local authorities. Food in damaged containers shall not be accepted, used or retained. This requirement is not met as evidenced by:Based on observation, the licensee did not comply with the regulation cited above. During inspection of the kitchen, LPA found damaged (burned) canned food in the pantry. This poses a potential safety, health, personal rights risks to persons in care.

  • Rapid-growth perishable foods must be covered

    All readily perishable foods or beverages capable of supporting rapid and progressive growth of micro-organisms which can cause food infections or food intoxications shall be stored in covered containers at appropriate temperatures. This requirement is not met as evidenced by:Based on observation, the licensee did not comply with the regulation cited above. During inspection of the refrigerator, LPA found half of an avocado that is not stored in a closed container. This poses a potential safety, health, personal rights risks to persons in care.

    Centrally stored medicines shall be kept in a safe and locked place that is not accessible to persons other than employees responsible for the supervision of the centrally stored medication. This requirement is not met as evidenced by:Based on observation, the licensee did not comply with the regulation cited above. During inspection of the kitchen, LPA found 2 medication pills in a drawer next to the dishwasher machine. These were not locked and were accessible to residents. This poses an immediate safety, health, personal rights risks to persons in care.

    the licensee shall ensure that disinfectants, cleaning solutions, poisonous substances, knives, matches, tools, sharp objects, and other similar items which could pose a danger to residents are in locked storage and are not left unattended if outside the locked storage. This requirement is not met as evidenced by:Based on observation, the licensee did not comply with the regulation cited above. During inspection of the kitchen, LPA found 2 knives in a drawer next to the oven. These were accessible to residents in care. This poses an immediate safety, health, personal rights risks to persons in care.

On 1/24/2026 resident (R1) eloped from the facility, without facility staff awareness, and sustained multiple injuries as they were struck by a vehicle. Which poses in immediate Health and Safety risk to residents in care.
