Inspector’s narrative

What the inspector wrote

On 9/26/2025 the Dept, reviewed R1’s financial transactions records, which indicated that R1 was not overcharged for board and care services during the applicable audit period of 07/08/2025 through 09/05/2025. Records review further indicated that the facility underwent a change of ownership in July 2025, with new ownership effective July 07, 2025. Due to the prior facility no longer being in operation, it could not be determined whether any overpayments were made before the change of ownership. This agency has investigated the complaint alleging that staff financially abused a resident. We have found that the complaint was unfounded, meaning that the allegation was false, could not have happened and is without a reasonable basis.
