What the inspector wrote

On 01/22/2026 Licensing Program Analyst (LPA) Shakaricka Hughes arrived unannounced to conduct a case management visit. LPA met with facility staff Enesi Yavala and explained the purpose of the visit. The facility administrator was not present during the visit. The current census is 5. The purpose of this visit was to speak with the facility Licensee/Administrator regarding discrepancies identified in the facility Admissions Agreement. During an investigation of a complaint that was filed with CCLD on 08/04/2025 regarding the facility, it was reported that an Admissions Agreement for a former resident contained inconsistencies regarding whether the resident paid board and care charges through private payer or SSI, and the Admissions Agreement contained missing required signatures. LPA attempted to speak with the facility administrator to highlight the errors in the Admissions Agreement and explain to the Administrator the requirements for completing the Admissions Agreement accurately and in its entirety, including clearly identifying the resident payment sources, and ensuring all required parties sign the document. An addition to reviewing Health and Safety §1569.884 Contents of residential care facility admission agreements to ensure clarity of documentation requirements. An exit interview was held with Enesi, and a copy of the report was provided to the facility.
