What the inspector wrote

An Non-Compliance Conference (NCC) meeting was held on 3/27/2026 at the Sacramento Regional Office via Microsoft Teams. The purpose of this meeting was to discuss concerns regarding both facilities. Participants in the meeting included: Regional Manager-Stephenie Doub, Licensing Program Manager LPM- Stephen Richardson, Licensing Program Manager LPM-Lisa Rios, Licensing Program Analyst LPA- Arvin Villanueva, Licensing Program Analyst LPA- Shakaricka Hughes, Licensing Program Analyst-Reza Jamaly, and Licensee/ Administrator- Veniana Banuve. During the meeting, an incident which occurred in the facility on 01/24/2026 was discussed and the following records for facility staff (S1) was requested by the Dept by 03/31/2026. Personnel Record for (S1) Training Records for (S1) This meeting has been rescheduled to an in-person office meeting on April 03, 2026 at 10:00 AM at the Sacramento Regional office. An informal invitation was sent via email to the licensee on 03/27/2026. This report was discussed with the licensee, and a copy of this report was provided to the licensee via email.
