What the inspector wrote

On April 23, 2026, Licensing Program Analyst, Arvin Villanueva(LPA), arrived unannounced at this facility to conduct the Post Licensing visit. This visit is being conducted concurrently with complaint investigation visits (Complaint #27-AS-20260414141418 & Complaint #27-AS-20260420105215). LPA initially met with staff on duty, Kirk Campbell (S1), and stated the purpose of the visit. Per S1, he notified the administrator but did not arrive during this visit. Upon arrival, LPA observed the fire door to be propped opened with a dining chair (photo taken). Later during the visit, LPA observed S1 removed the dining chair and closed the fire door. During this visit, LPA conducted review of two resident records. LPA conducted facility observation. The sliding door, marked exit, by the kitchen/living room area was difficult to open. It required LPA a bit of a force to slide it open. Advisory was given to ensure the door is easy for residents to slide it open. Based on today's visit, this post licensing inspection requires continuation. The Department will return at a later date to continue the inspection. Deficiencies are being cited today. Immediate Civil Penalties is assessed today in the amount of $1000 due to repeat fire safety violation. Exit interview was conducted a copy of this report and appeal rights were provided.

    Maintain facilities for fire and panic safety

    Fire Safety: All facilities shall be maintained in conformity with the regulations adopted by the State Fire Marshal for the protection of life and property against fire and panic. Based on observation, upon arrival, LPA observed the fire door was propped open with a dining chair. This poses an immediate health, safety, and personal rights risks to persons in care.
