Based on staff interviews, observation, record review, facility did not comply with Regulation as staff was not administering resident, R2s medications per their doctor's orders, ( order-Vitamin D (2000IU) staff giving Vitamin D (5000IU), (order -Metoprolol-50MG, staff giving Metoprolol-25MG ), (order Pantoproazole 40mg, staff giving Pantoproazole 40mg) and no updated medications list to review for resident R2, which poses an immediate health, safety or personal rights risk to persons in care.

Based on staff interview and record review, resident, R2-medications were found to be in pharmacy bubble pack from March 1-6, 2024 during medications audit. Facility was not able to answer, how R2 was getting the medications from March 1-6, 2024, which poses an immediate health, safety or personal rights risk to persons in care.

Based on staff interview and record review for resident , R2, facility did not have updated LIC602 and Re-appraisal with dementia diagnosis as required by this regulation which poses a potential health, safety or personal rights risk to persons in care.

87506 Resident Records (c) All information and records obtained from or regarding residents shall be confidential. (1) (…) The licensee and all employees shall reveal or make available confidential information only upon the resident's written consent or that of his designated representative. This requirement is not met as evidenced by: Based on records reviewed, the facility did not ensure that resident R1’s records were made available via request by relevant party with authorization of R1’s responsible party, which poses a potential health, safety, and personal rights risk to residents in care.

87457(c)-Prior to admission a determination of the prospective resident's suitability for admission shall be completed and shall include an appraisal of his/her individual service needs in comparison with the admission criteria specified in Section 87455, Acceptance and Retention Limitations. This requirement is not met as evidenced by:Based on record review and interviews, Licensee did not do pre admission or new admssion assessment for R1 at time of R1 admission to the facility on/before 03/27/22, which poses a potential health and safety risk to residents in care.

87705- Care of Persons with Dementia (c) Licensees who accept and retain residents with dementia shall be responsible for ensuring the following: (4) There is an adequate number of direct care staff to support each resident’s physical, social, emotional, safety and health care needs as identified in his/her current appraisal. This requirement is not met as evidenced by: Based on interviews conducted and record review , the Licensee did not ensure that resident (R1) was unable to leave the facility unassisted, on 03/28/22, which posed an immediate health and safety risk to residents in care.

Maintenance and Operation - The facility shall be clean, safe, sanitary and in good repair at all times. The licensee did not ensure that the facility was safe from pests and rodents.

Based on LPAs' observations during visit, the facility did not ensure that knives were locked and inaccessible to residents, which poses an immediate health, safety or personal rights risk to persons in care.

During visit, LPAs observed debris in backyard. Based on observations, the facility did not ensure that backyard was clear of debris, which poses/posed a potential health, safety or personal rights risk to persons in care.

During visit, LPAs observed food supply with multiple expired items, open packages, and food items without date or label. Based on observations, the facility did not ensure residents had a sufficient supply of food, which poses an immediate health, safety or personal rights risk to persons in care.

Based on LPA's observations, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Facility does not document daily temperature and COVID-19 symptom checks, and/or any change in condition for staff and residents in order to track spread and why facility took certain steps to prevent and mitigate spread in the facility. In addition to this requirement, LPA observed S2 did not wash her hands after using the restroom.

Based on LPA's observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. A sign-in policy is to be enacted with all visitors to ensure compliance with central entry point for symptom screening and to record contact information (for reporting requirements to public health officer and contact tracing).

Based on LPA's observations and interviews, the licensee did not comply with the section cited above which posed a potential health, safety or personal rights risk to persons in care. LPA observed cockroaches throughout facility. Licensee stated that attempts have been made to treat the pests with over-the-counter sprays such as "Raid". However, no professional Pest Control services have been utilized. LPA observed that after incontinence care, soiled resident diapers are being openly discarded in trashcans without lids in the bathrooms.
