Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a case management inspection following the issuance of a Decision and Order on 7/14/2022. LPA met with Bill Phelps, Interim Administrator, and explained purpose of inspection. Prior to initiating today's inspection, LPA completed required COVID-19 testing protocols, contacted the facility to confirm there are currently no positive Covid-19 diagnoses, completed a daily self-screening questionnaire for symptoms of COVID-19 infection to affirm no COVID-19 related symptoms. Additionally, LPA was screened per Covid-19 precautionary measures upon entering the community via Accu-Shield system. LPA ensured she applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: N95 mask. LPA discussed Decision and Order issued on 7/14/22. A copy was not received by the facility. Proof of Service does not indicate a copy was sent to the facility or facility headquarters. LPA printed a copy of the Decision and Order during today's inspection and emailed a copy following the inspection. Interim Administrator confirmed through Business Office Director that individual #7517215841 last worked at the facility on/around October 2020 and was disassociated from the facility on 11/6/2020. There are no deficiencies issued during today's inspection. Exit interview. Copy of report provided to Administrator.
