This requirement is not met as evidenced by: Based on record review, the Licensee did not ensure that staff recognized the effects of medications commonly used to treat symptoms of dementia and provided resident (R1) with sufficient observation, on 5/26/2020, to ensure his safety needs were met, which posed an immediate health and safety risk to resident in care. Resident fell on 5/26/2020 at approximately 10:45 pm and sustained a serious head injury and passed on 5/31/2020.
