Licensing Program Analyst (LPA) Magda Malcore conducted an unannounced visit to the facility to conduct a health and safety check. LPA was granted entry and met with staff, Yanira Saldivar. The facility has a current census of two (2) resident in care. No residents are currently receiving hospice services. The health and safety check consisted of facility tour, observation of resident medications, telephone service, food supply, physical plant, and residents. LPA observed resident #1 (R1) in their bedroom and resident #2 (R2) was observed walking around inside the facility before returning to their bedroom. No deficiencies were cited during today's visit. An exit interview was conducted where this report was discussed and a copy provided to staff Saldivar at the conclusion of the visit.
