87156 Licensing Fees(a) An applicant or licensee shall be charged fees as specified in Health and Safety Code section 1569.185. During a record review, LPA observed the facility has not paid their annual licensing fees due on 7/12/2024 and has a current balance of $4,455.00. This poses a potential health or safety risk to clients in care.

87303 Maintenance and Operation(a) The facility shall be clean, safe, sanitary and in good repair at all times. Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors. Based on observations, interviews conducted, and record review the facility roof is in disrepair as water continues to leak throughout different areas of the facility, including R1 and R2's room. This poses a potential health, safety, or personal rights risk to residents in care.

87555. General Food Service Requirements. (b) The following food service requirements shall apply:(9) Procedures which protect the safety, acceptability and nutritive values of food shall be observed in food storage, preparation and service. Based on interviews and records review, facility did not observe procedures to protect the acceptability of food, which posed a potential health risk to 1 out of 150 residents in care.

87468.2. Additional Personal Rights of Residents in Privately Operated Facilities. (a) In addition to the rights listed in Section 87468.1, Personal Rights of Residents in All Facilities, residents in privately operated residential care facilities for the elderly shall have all of the following personal rights:(8) To be free from neglect, financial exploitation, involuntary seclusion, punishment, humiliation, intimidation, and verbal, mental, physical, or sexual abuse.Based on interviews and records review, staff #1 financially exploited R1 which posed a potential personal rights risk to 1 out of 150 residents in care.
