What the inspector wrote

Licensing Program Analyst (LPA) Correia conducted an unannounced Case Management visit to issue a deficiencies. Upon arrival, LPA Correia was greeted by care staff Charles Oronan, identified herself, was granted entry, and met with Licensee Seiko Iso. During an unrelated facility visit, it was discovered that an individual was residing in an Accessory Dwelling Unit (ADU) located on the facility grounds. A review of facility records revealed that the individual was not associated with the facility. Additionally, interviews with an outside source and the Licensee confirmed that the Licensee did not meet reporting requirement per State mandate. Based on interviews and a review of records, violations are being cited per Title 22, Division 6, Chapter 1 of the California Code of Regulations and are listed on the LIC 809-D. An immediate civil penalty of $500 was assessed and documented on the LIC 421IM. An exit interview was conducted, and a copy of this report, along with Licensee Rights (LIC 9058 03/22) , was provided to Licensee Iso. Licensee Iso’s signature below confirms receipt of these documents.
