What the inspector wrote

Regional Manager (RM) Jerry Romero and Licensing Program Manager (LPM) Lizzette Tellez conducted an Office Meeting with the Co-LIcensee, Frank Ramirez, and their Attorney, Jesus Partida. RM explained the purpose of the meeting. On today's date, the RO and Mr. Ramirez and Mr. Partida met to discuss the licensee's control of property and current status. Licensee provided legal documentation noting dismissal of an eviction notice. The Licensee was provided with a copy of CCR 87211 Reporting Requirements. Licensee agreed to provide updates to CCL regarding the licensee's operating status and control of property issues. An exit interview was conducted. A copy of this report, along with Licensee/Appeal Rights, were provided to Mr. Ramirez at the conclusion of the visit. His signature on this form acknowledges receipt of these rights.

This requirement was not met as evidenced by: Based on an interview with the Licensee there was a individual residing on the facility ground that was not associated to the facility.This posed an immediate threat to 14 out of 14 residents in care.
