Based on record review, the licensee did not comply with the section cited by not ensuring that staff 1 (S1) 8 hours of in-service training per year on the subject of serving residents with dementia. Based record review 4 out of 5 resident has dementia, which poses an immediate health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply by not ensuring that 4 out of 5 residents who have dementia are appraised once every 12 months. The appraisal for 3 out of 5 was dated 2017 and 2018, 2023, and 2024, 2022, & 2 out of 5 does not have any appraisal on record, which poses potential health, safety or personal rights risk to persons in care.
