What the inspector wrote

Licensing Program Analyst (LPA) Maria (Mita) Partoza conducted an unannounced annual required inspection and met with licensee/administrator (LIC/ADM) Diana Natividad and stated the purpose of the visit. The facility is licensed for adults 60 and over and all 6 may be non-ambulatory. LPA observed 4 residents were present and were at their respective rooms and 1 staff. LPA toured the facility, including common areas, resident rooms, kitchen, bathrooms, driveway, and outdoor spaces and storage areas. Indoor temperature was within acceptable range of 70°F. The kitchen was sanitary and organized; knives and chemicals were locked. Food supply met requirements (2 days perishable, 7 days non-perishable). Kitchen water temperature measured at 118.7°F. Bathroom water temperature is at 117.8°F. Bathrooms had grab bars and non-skid mats, however, LPA observed the visible debris and brown stained buildup observed within the toilet bowl below the waterline . Resident rooms had adequate storage. Medications were locked and inaccessible to residents. The outdoor walkway are free from obstruction and any tripping hazards; laundry appliances were functional, and cleaning supplies were secured. Fire, smoke, and carbon monoxide systems were operational; hallways were clear and well-lit. page 1 of 2 LPA reviewed resident and staff records, including medication logs, admission agreements, care plans, personal and incidentals, health screenings, and training. Based on document review the following were observed; the appraisal needs and services plan for 1 out of 5 done on 2017, and 2018, 1 out of 5 was done on 2023, and 1 out of 5 done in 2022 and 2 out of 5 had no appraisal needs and services plan. 1 out of 2 staff training was dated 2024, 2019 and 2013. Based on record review 4 out of 5 residents were diagnosed with dementia. The facility conducted the fire drill 3/01/2026. According to licensee, the facility conducts fire drills but not for earthquake drills. The facility is equipped with panel fire alarm system and 3 fire extinguisher were inspected on 04/01/2026. Deficiencies were cited during today's visit based on the California Code of Regulations (CCR) Title 22 See LIC 809D for details. The appraisal for 3 out of 5 was dated 2017 and 2018, 2023, and 2024, 2022, & 2 out of 5 does not have any appraisal on record, which poses potential health, safety or personal rights risk to persons in care.
