During review of 5 out of 5 resident centrally Stored Medication and Destruction Records(CSMDR), the Licensee did not ensure that resident R1’s CSMDR did not have 7 missing medications, which poses a potential safety risk to residents in care.

87202 (a) All facilities shall maintain a fire clearance approved by the city, county, or city and county fire department, or district providing fire protection services, or the State Fire Marshal.This requirement is not met as evidenced by: Based on observation, LPAs observed a mattress and S2's medications in the office area. S1 and S2 stated S2 is a live-in staff and uses the office area as a bedroom which poses/posed an immediate Health, Safety, or Personal Rights risk to persons in care.

This requirement is not met as evidenced by: Based on observation, Licensee did not ensure food stored at the facility were not expired wherein LPA Rai observed 4 out of 10 canned foods and packaged food were expired which poses/posed an immediate Health, Safety, or Personal Rights risk to persons in care.

87608 Postural Supports(a)(5)(B) Bed rails that extend the entire length of the bed are prohibited except for residents who are currently receiving hospice care and have a hospice care plan that specifies the need for full bed rails.This requirement is not met as evidenced by: Based on observation, LPAs observed resident R2 using a full bed rail and S1 confirmed R2 was not on hospice services which poses/posed an immediate Health, Safety, or Personal Rights risk to persons in care.

Based on observation, interview and record review, the licensee did not comply with the section cited above in 3 medications for R2 were expired in the medication box and not recorded in the medication dustruction log and still in the bubble pack container which poses/posed a potential health, safety or personal rights risk to persons in care.

Regular observation and documentation of resident changes

Based on record review, the licensee did not comply with the section cited above in 2 out of the 2 residents did not have record of weight log in file. R3 was weighed during visit and R6 is bedridden and other means of measurement was not recorded which poses/posed a potential health, safety or personal rights risk to persons in care.

Onsite food stock levels for nonperishables and perishables

Based on observation and interview, the licensee did not comply with the section cited above in facility did not maintain one week of non perishable supplies and LPA Rai observed (count) canned fruits and vegetables which poses an immediate health, safety or personal rights risk to persons in care.

No admission for total dependence in daily living tasks

Based on observation, interview and record review, the licensee did not comply with the section cited above in 1 out of 6 resident requires total care which includes resident not able to reposition in bed without assistance which poses an immediate health, safety or personal rights risk to persons in care.
