What the inspector wrote

Licensing Program Analyst (LPA) Christine Dolores arrived unannounced to conduct a case management visit regarding multiple incidents on theft that were reported to our Department. LPA met with Executive Director (ED), Lauren Powell. From 02/28/2023 - 03/13/2023, the Department was reported 4 incidents of theft at the facility involving residents R1 - R4. The residents and responsible parties were all aware of the incidents. The facility reported the incidents to the police department and Ombudsman. On 03/08/2023, the facility held a town hall meeting with the residents and family, to discuss topics included but not limited to the activity of theft occurring at the facility. The facility recommended the residents to secure possessions such as checkbooks, credit cards, and other valuable items. The facility provided certain residents with a lockbox to secure valuable items. The residents were informed to report any missing items, as soon as possible. LPA and ED discussed an action plan to ensure the residents are kept safe during ongoing investigation. During visit, LPA entered 3 out of 4 resident apartments with ED. LPA interviewed R1, R2, and ED. The following documents were obtained to include the resident roster, staff roster, staff schedule from 02/27/2023 - 03/19/2023, training records on theft, facility's policy and procedures on theft, R1 - R4's physicians reports, appraisal needs and services plan, and staff (S1) employee application. No deficiencies were cited per California Code of Regulations, Title 22. This report was reviewed with Executive Director (ED), Lauren Powell and a copy of the report was provided.
