What the inspector wrote

Licensing Program Analyst (LPA) Manuel Monter arrived unannounced to deliver the results of a complaint investigation 26-AS-20240613120601. During the complaint investigation, a case management deficiencies visit was conducted due to violations discovered during the investigation process. LPA met with Administrator Meghian Geul. On September 30, 2025 the Department requested copies of R1’s Admission Agreement, documentation showing the facility notified R1’s responsible party of the rate increases. As of November 20, 2025, LPA has not been provided these requested documents to review and create copies. On November 20, 2025, LPA interviewed ADM Meghian Geul. LPA asked ADM if she can provide the requested documents to review. ADM stated R1 moved away from the facility on June 16, 2024. ADM stated the only documentation the facility has for R1 is his/her physicians report, care plan, and assessment Deficiencies are being cited per California Code of Regulations, Title 22. See LIC809-D. This report was reviewed with administrator Meghian Geul and a copy of the report and appeal rights were provided.
