Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Case Management visit and met with Racheal Mosely. The purpose of the visit was to amend reports that were originally delivered to the facility on 05/28/2025 and 08/22/2025. The report from 05/28/2025 was part of a complaint investigation that the department received on 05/12/2025. The report from 08/22/2025 was part of a complaint investigation that the department received on 05/06/2025. During the investigations, LPA Marrufo reviewed the Appraisal/Needs and Services Plan for resident R1. LPA Marrufo observed that R1's Appraisal/Needs and Services Plan was not written with needs and objectives that are measurable and verifiable. An advisory note was issued. LPA Marrufo advises facility Administrator to review the information about the Department’s Technical Support Program (TSP) found at the Community Care Licensing Division (CCLD) website: www.cdss.ca.gov . No deficiencies were cited at this time as per California Code of Regulations Title 22. This report was reviewed with Racheal Mosely and a copy of this report was provided.
